Creatine After Menopause: What Does It Have to Do With Stronger Bones?

When most people think about creatine, they think about muscle.

More strength.
Better workouts.
More power.

But as women move through menopause, the conversation around strength becomes much bigger than aesthetics or athletic performance.

It becomes about maintaining muscle, maintaining strength, staying active—and protecting the physical foundation that allows you to stay independent as you age.

And that is where creatine gets particularly interesting.

Research has been investigating whether creatine supplementation, when combined with resistance training, may help support bone health in postmenopausal women.

One randomized controlled trial published in Medicine & Science in Sports & Exercise produced some particularly interesting results.

The Muscle-Bone Connection

Your skeleton isn't just a rigid framework holding you upright.

Your bones are living tissue that responds to mechanical forces.

When you perform resistance training, your muscles pull against your bones. Those forces provide a signal that can help stimulate the processes involved in maintaining bone structure.

That's one reason resistance training is so important as we age.

But there's another piece to the equation:

How strong are you?

The stronger your muscles become, the greater the loads you can safely place on your body during training.

Creatine may help here by supporting the phosphocreatine energy system, allowing your muscles to regenerate ATP more rapidly during high-intensity activity.

In other words, creatine doesn't need to directly "build bone" to potentially contribute to better bone health.

It may help you train harder and become stronger—which changes the mechanical stimulus placed on your skeleton.

And researchers wanted to know whether that translated into measurable changes in bone.

The Study: Postmenopausal Women + Creatine + Resistance Training

Researchers recruited 47 postmenopausal women with an average age of approximately 57.

The women were randomly assigned to receive either creatine or a placebo while participating in a supervised resistance-training program three days per week.

The creatine dosage was approximately 0.1 grams per kilogram of body weight per day.

After 12 months, researchers evaluated bone mineral density, bone geometry, muscle mass and strength.

And that's where things got interesting.

 

Creatine Helped Preserve Femoral Neck Bone Density

The women taking creatine experienced a smaller decline in bone mineral density at the femoral neck than the women taking placebo.

The femoral neck is particularly important because it is part of the hip and is a common location of concern for bone loss as women age.

The creatine group experienced approximately a 1.2% decrease in femoral neck bone mineral density compared with approximately 3.9% in the placebo group over the 12-month intervention.

That's not the same thing as saying creatine "increased bone density."

It didn't.

The more accurate interpretation is that creatine attenuated the loss of femoral neck bone mineral density during the year-long resistance-training program.

That's a subtle distinction—but an important one.

The Bones May Have Become Structurally Stronger, Too

Researchers also found an increase in femoral shaft subperiosteal width in the creatine group.

That may sound like a mouthful.

Here's why it matters:

Bone strength isn't determined solely by how much mineral is packed into the bone.

The size and geometry of the bone also influence its ability to resist bending forces.

The researchers described the change in femoral shaft width as a predictor of greater bone bending strength.

So the findings weren't simply about a number on a bone-density scan.

They suggested that creatine plus resistance training may influence certain structural characteristics of bone.

And Then There's Strength

Perhaps unsurprisingly, the women taking creatine also saw a greater improvement in relative bench-press strength.

The creatine group improved approximately 64%, compared with 34% in the placebo group.

And this may be one of the most important pieces of the story.

Because strength isn't just about lifting heavier weights.

Strength is what allows you to:

  • Carry groceries
  • Get up from the floor
  • Climb stairs
  • Pick up your grandchildren
  • Maintain balance
  • Continue doing the activities you enjoy

The goal isn't simply to become stronger for the sake of being stronger.

It's to remain capable.

Why This Matters After Menopause

Menopause brings significant changes to the body.

Muscle mass and strength can decline with age, while bone density can also decrease.

That combination can create a frustrating cycle:

Less muscle → less strength → less physical activity → less mechanical stimulus → greater loss of physical capacity.

Resistance training can help interrupt that cycle.

And creatine may be one tool that helps women get more from their training.

Importantly, creatine isn't a replacement for resistance training.

The training is still the foundation.

The 2015 study specifically investigated creatine alongside a supervised resistance-training program.

But What Does the Newer Research Say?

This is where it's important to look at the entire body of evidence—not just one exciting study.

A 2026 systematic review and meta-analysis examining seven randomized controlled trials involving 608 postmenopausal women found that creatine supplementation was associated with modest improvements in lean mass and meaningful improvements in strength, particularly when combined with resistance training.

However, the researchers found that bone mineral density was unchanged overall.

Their conclusion was that the effects of creatine on bone density remain unclear.

There's also a two-year randomized controlled trial involving 237 postmenopausal women that found creatine combined with resistance training and walking did not significantly improve bone mineral density at the femoral neck, hip or lumbar spine compared with placebo.

So we shouldn't tell women that creatine is a bone-building supplement.

The evidence doesn't support that claim.

What the evidence does support is much more interesting:

Creatine can help support muscle and strength, and resistance training is one of the most important things women can do to maintain musculoskeletal health as they age. 

Think Beyond Bone Density

This may be the biggest takeaway.

Healthy aging isn't about protecting one isolated measurement.

It's about maintaining the ability to move, lift, carry, climb, balance and participate in life.

That requires muscle.

It requires strength.

It requires physical activity.

And it requires bones capable of supporting all of it.

Creatine sits within that bigger picture.

Rather than thinking of creatine as something reserved for bodybuilders or twenty-year-old athletes, think of it as a nutritional tool that may help support strength and physical performance throughout the decades of life when maintaining those qualities becomes increasingly important.

The Takeaway

Creatine began its reputation as a sports-performance supplement.

Today, researchers are investigating its potential role in healthy aging, cognitive performance, muscle preservation and bone health.

For postmenopausal women, the most compelling evidence currently points toward its ability to support strength and lean mass, particularly when paired with resistance training.

The bone-health research is promising in some studies but remains mixed overall.
